Authorization to Release Information
- This form is required to be submitted to CBA Blue in certain circumstances when someone other than the subject of the PHI is requesting a release of information.
Confidential Communications Request
- This form is required to be submitted to CBA Blue by any Individual requesting alternative treatment of communication of their Protected Health Information.
Amendment Request
- This form is required to be submitted to CBA Blue by any Individual requesting an amendment to their Protected Health Information.
Access Request
- This form is required to be submitted to CBA Blue by any Individual requesting access to their Protected Health Information.
Disclosure Accounting Request
- This form is required to be submitted to CBA Blue by any Individual requesting an accounting of disclosures made by or on behalf of CBABlue Protected Health Information.
